The Department of Public Health and Social Services Division of Environmental Health has begun accepting renewal applications for sanitary permits.
Applications are available at the DEH office located on the second floor of the Central Public Health Facility in Mangilao. The forms also can be downloaded from the department's website at http://www.dphss.guam.gov.
Completed renewal applications that will be paid by check may be dropped off at the DEH office from 8 a.m. to 4 p.m. Monday through Friday. A copy of the current sanitary permit must be attached to the renewal application. The applicant will be notified by phone when the permit is available for pickup.
The fee for the issuance of a sanitary permit for health-regulated establishments with up to 10 employees is $290, and $5 for each additional employee thereafter.
It is unlawful for any person to operate or open a health-regulated establishment without a valid sanitary permit, officials stated. Businesses are encouraged to pick up renewal applications on or before 4 p.m. Friday.
